Here's the
index of online psychological questionnaires, surveys and quizzes. Some
are for research purposes, some are for fun. Please note that none of
these questionnaires is formally standardised and do not constitute
psychometric tests. They are merely intended as rough guides. If you would
like to undergo more formal psychological testing then please contact one
of the professionals in the Find a psychologist section of the site. Check
this page often because there will be frequent new additions.
Psychoactive
Medication Survey - Participate in an informal survey of the user's
experience of psychoactive medications. If you are or have at any time
taken any psychiatric drugs - that's anti-depressants, anti-anxiety,
anti-psychotic, mood stabilisers - in short any scheduled psychiatric
medication then please take 3 minutes to complete this 13 question survey.

Zung
Self Report Depression Scale - This
scale has been adapted from the widely used Zung scale first published in
the Archive of General Psychiatry in 1965. (Volume 12, pages 63-70).
Inventories are limited as a way of describing human experiences but this
can be a useful way for you to assess the extent to which your mood-space
is impacting on your life. Please note that this questionnaire was
originally designed to be used in conjuction with consulation with a
mental health professional.
Zung
Self Report Anxiety Scale - Again,
this scale adapted from the one by Dr Zung. Rate yourself on the 20 items
it presents. Questionnaires generally deliver results that are defined by
the point of departure taken when they were designed. While this survey
can in no way be used to provide you with a diagnosis or a course of
treatment it can serve as a tool to focus your attention on the extent to
which anxiety and worry are factors influencing your life-space.
All responses to these questionnaires are confidential.
